Measure your sister’s tommy and let every lady measure her tommy with toilet paper and the person who is more close to it wins, also buy different chocolates and melt them in microwave and put them in several baby pampers and then pass it around and the ladies taste it and have to say what kind of chocolate it is, and is funny because you don’t say what it is and when they taste it out of the pamper it looks like baby poop, also buy a toy baby and let every lady put a diaper to the toy and the person who finish first and the pamper correctly wins, you can also put them into teams of two persons and one will be the mom and the other the baby and the one playing the mom have to feed the other person who is playing as a baby a gerber and have to give it to her with a spoon and she needs to wear a bib and the team that finish first wins they have to eat all the gerber.

you shouldn’t need anymore food for the shower.
you could play baby bingo. all you have to do is give them a blank bingo card and have them fiill in items you think sister may get as gifts for the baby. then as she opens the gifts, have them mark on their card what was just open.
or you could play this: get a bunch of blue/pink clothes pins and give each guest one on their shirt as they come in. throughout the party if they say the word, "baby" their pin gets taken away by the person who caught them saying it, and the person who caught them gets the pin. at the end, the person with the most pins wins.
